Muscle loss on GLP-1 treatment: prevention that works
Up to a third of weight lost can be muscle if you do nothing. The two-lever fix: protein and resistance training.
The reality, unflinching
Rapid weight loss without protection costs muscle, in GLP-1 trials, lean mass accounted for roughly 25 to 39% of total loss where no countermeasures were taken. That is not a reason to avoid treatment. It is a reason to pair it with the two levers that change the ratio.
Lever 1: protein
1.2 to 1.6 g per kg of target weight, every day, spread across meals. The protein guide has foods, grams and tactics for small appetites. Without adequate protein, training has nothing to build with.
Lever 2: resistance training
Muscle that is used is muscle that is kept. Two to three sessions a week of progressive resistance, weights, bands, or bodyweight, is the signal your body needs to prioritise fat loss over muscle loss.
- Prioritise compound movements: squats, hinges, pushes, pulls.
- Two sets per movement is enough at the start. Add load gradually.
- Thirty minutes counts. Consistency beats intensity.
How to know it is working
The scales cannot separate fat from muscle. Better signals: strength holding or rising in your sessions, waist shrinking faster than weight falls, and energy staying good. Some people use gym-based composition scans for reassurance, useful, but trends in strength and waist tell you most of what matters.
Extra care over 50
Muscle is harder to build and easier to lose with age (sarcopenia), so the protein target sits at the top of the range and strength work becomes non-negotiable rather than optional. If you are over 60, tell your prescriber, dose pace may be gentler to slow the rate of loss.
